Individual Treatment Plan (ITP)

The Individual Treatment Plan complies with Minnesota rules governing the plan of care in a recipient's health record. This plan also complies with specific requirements for ARMHS, ACT and IRTS found in Minnesota Statues. The ITP incorporates clinical as well as rehabilitation-focused plans.
