Special Issues for IRTS
Goals
answer the question, “What functioning must the person be able to
do, to successfully engage with their post-IRTS treatment/rehab services
and environment?”
Here are some examples:
- I will be able to call my doctor’s office before I stop taking my medicine.
- I will be able to have three different people, including my ARMHS worker, whom I can ask for help.
For persons who are admitted to an IRTS from an ARMHS or ACT team, the IRTS staff should incorporate the person’s recovery goals and major rehabilitation goals from their ACT or ARMHS ITP into the IRTS planning process with the person.
While these will rarely be achieved within an IRTS, the person will continue to work on these in their next level of care. Referencing these goals helps the person to see their stay in the IRTS as one step in their recovery and assists in building continuity of care across services.
