A number of assessment forms and/or tools are available to support the assessment process. Some are developed by an individual agency for use solely with the agency’s clients. Other forms and standardized tools of assessment are validated and reliable. Some assessment tools will be broad and others will focus on a single area of functioning, such as vocational assessment, coping skills assessment, social skills assessment or cognitive functioning.

Minnesota does not require use of a specific assessment tool.  Talk to your supervisor about if your agency uses any standardized assessment tools or formats.
