Plan monitoring, evaluation and revision

Plan monitoring, evaluation and revision are inter-related processes during which case managers verify on a regular basis that:

  1. Services identified in the plan are delivered.
  2. Services are of sufficient type, quality and amount to meet health, safety and welfare needs.
  3. Outcomes identified in the plan are met.
  4. Plans reflect current needs.
  5. Persons are offered choice among providers of service.

While plans have mandated, minimum timelines for review, case managers should evaluate plans frequently, both formally and informally, and modify them when needed.
