Fundamentals of Case Management
The e-learning course, Fundamentals of Case Management was designed for the beginning learner who desires to develop an initial understanding of case management in Minnesota.
The course, Fundamentals of Case Management presents the following content in three lessons. Each lesson may take the learner an hour to complete.
Lesson #1: Overview. This lesson presents general information for beginning case managers working in the state of Minnesota. It describes the roles and principles of case management and presents information on the laws applicable to case management and introduces the case management process.
Lesson #2: Intake, Eligibility Determination and Screening. This lesson presents information on the first three parts of the case management process and discusses intake, eligibility determination and screening in more detail than lesson one.
Lesson #3: Service Plan Development, Service Authorization & Monitoring Implementation. Lesson three continues with the case management process and presents the three final topics: service plan development, service authorization and monitoring.
Note to the learner: At the beginning of each lesson you will find a list of goals and objectives. The content of each lesson and the assessment portion of the lesson are based on these goals and objectives.
