Case Management in Minnesota Plan development, Service Authorization
and Plan Implementation

Plan development: Case managers use assessment information to help persons select services based on their needs, eligibility, cost effectiveness and preferences. Formal plans of service, signed by service recipients, legal representatives and case managers, identify the type, amount, duration and scope of services to be provided.

Service authorization: Case managers follow county procedures to enter services identified in the formal plans of service into the Minnesota Medicaid Management Information System (MMIS).

Plan implementation: Case managers help persons to identify providers, initiate and coordinate authorized services.
