Fundamentals of Case Management: Intake, Eligibility Determination, Screening
The intake process directed by case managers occurs in three separate but interrelated steps:
Intake: Case managers initially interview persons to explore service requests and initiate case files.
Eligibility determination: Case managers obtain new assessments and review information from previous assessments and/or historical records to identify areas of need and apply program criteria to determine if eligibility requirements are met.
Screening: Case managers determine levels of care for persons who request specific services such as home and community-based waivers or institutional placement by addressing needs, diagnoses and functional capacities. The screening process may or may not occur during the intake meeting.
