Fundamentals of Case Management: Eligibility determination

Eligibility determination is the next step in the intake process. Case managers determine if persons are eligible for particular programs and/or services.

Whether or not persons receive services and the types of services they receive is based on eligibility criteria specific to each program. Persons who do not meet program requirements are not eligible for the programs. For example:

Program/service type

Eligibility base

Medical Assistance (MA - Medicaid)

Financial need

Medicare services

Age and/or diagnosis

Social Security Disability Benefits (SSI, RSDI)

Disability

Development Disability Rule 185 Case Management

Diagnosis, adaptive functioning and functional limitations

Minnesota Disability Health Options (MnDHO)

Medical Assistance eligible
