Fundamentals of Case Management: Eligibility determination

Conducting assessment and gathering information

Medical Assistance health and human service programs require assessments and/or diagnostic evaluations to determine persons' eligibility for services and identify areas of need. Case managers will often facilitate and coordinate the completion of assessments.

There are a number of processes that collect information to help formulate and arrive at informed decisions.

Formal diagnostic assessments: Results from specific measures, ratings and scores of targets based on standardized tests, inventories and evaluations. Often, these assessments are those only licensed medical doctors or licensed psychologists can provide. Examples include intelligence tests, behavior scales and medical evaluation tests.
