Plan development: Person-centered planning

Person-centered planning is a method used to help persons exercise self-determination in the planning and implementation of the services needed to keep them safe and healthy.

The process of developing service plans must be person-centered. Person-centered planning uses groups of people to focus on a person and the person's vision of what they would like to do in the future. The person is the focus of the planning and directs development of the plans with support from people who know him or her the best.

Through person-centered planning, persons are able to identify and develop plans that help them achieve their goals and dreams. Case managers use these goals and dreams to identify measurable and objective goals and outcomes that become part of the plans.

Planning through the person-centered process must address all assessed needs of the person.
