Plan development: Meetings
Conducting and recording outcomes of meetings requires good organizational, listening and facilitation skills. Two to three weeks before a meeting, a case manager should notify all required participants. The meeting notices should include information and a list of issues for discussion. This allows participants time to prepare comments and questions.
Case managers gather information from a number of people and may need to resolve disagreement among planning team members before they can create written plans that relate to persons' assessed needs for services and supports.
During meetings, case managers may need to keep the discussion focused and should have a method to track decisions made and flag items for follow up. It may be necessary to ask questions and determine if information shared is understood by all present. Provide interpreters and information in alternate formats and/or languages as needed.
Case managers should complete the services plans as soon as possible after meetings to record the information while it is still fresh. Keep in mind who will be using the service plans and document in easy to read language and avoid technical jargon.
