Plan implementation

Once plans have been developed and services authorized, plans are implemented. Case managers help persons identify providers, initiate and coordinate authorized services. Providers must hold and maintain required licenses for the provision of services.

When identifying providers, case managers select from providers who have contracts with the county. If no providers that offer the needed services are contracted with the county, case managers develop a plan for obtaining the services.

Case managers select providers that can provide services as described in the service plans. This means the providers are able to:

  • Maintain persons' health and safety
  • Comply with required reporting, communication, and service coordination requirements
  • Maintain compliance with applicable regulations and provide services in a cost-effective manner.
