Plan implementation

The written service plans guide providers as they develop their plans using information from the plans developed by the counties. Encourage providers to use person-centered planning processes to discover more about the person and his or her dreams, needs, and strengths as they develop their plans.

Case managers must take responsibility for keeping service plans updated to reflect current needs through monitoring and screening activities. However, many times, service providers have similar requirements to keep their required services plans updated as well. Case managers assure the needs identified through service planning are met and the information and updates are timely.
