Plan monitoring
Monitoring is on ongoing process case managers use to determine service effectiveness. Monitoring typically occurs on a fixed schedule and as needed. Case managers define how often services will be monitored based on persons needs and document schedules in the plans. Statues and rules for many programs and services define minimum timelines and methods of monitoring.
In addition to meeting regularly with the screening team and/or persons and their families, monitoring can occur through the review of documents, service narration, incident or medical reports, program progress and summaries. Monitoring may occur over the telephone, through email and written contact with consumers, family members, friends, service providers and medical professionals.
With all monitoring, it is important to take good notes. All notes should be dated and contain the type of contact (in person, telephone, email), time of contact, name(s) of the person(s) contacted and a description of the situation.
