Evaluation is the process when case managers take information/data gathered through the monitoring process and measure progress against the outcomes and goals previously identified. Decisions are made about outcomes, both intended and otherwise, and changes needed to better address the needs of the persons.

An important role of case managers is to integrate the individual parts of monitoring into a compatible whole. In addition to looking at the delivery of services, identify what has gone well. Has something been neglected? Is there any duplication of effort?

Interactions with service providers during the monitoring process provide case managers a basis for deciding which contracts to renew, expand, revise or terminate when negotiating services with providers. It is important to recognize good performance, and keep interest and enthusiasm high. Through a variety of techniques, case managers direct the team efforts towards the goals and outcomes identified in the planning process.
