Plan revision

The final process in this module is the revision of plans. Case managers revise plans to document decisions that result from the monitoring and evaluation processes.

Case managers meet with persons and their representatives to discuss current levels of service needs and prioritize service needs within the service plans. This revision process may include renegotiating, setting realistic timelines, resolving differences, solving problems and redefining parts of the plans.

In most instances, service providers will not be present at the planning meetings. Information required of providers may be obtained prior to the meeting or as a follow-up to the meeting.

Plan revision includes any of the following:

  • Amending existing outcomes, goals and services to be more realistic
  • Creating new outcomes, goals and services
  • Changing how implementation of any of the above may occur.
