Promotion People Process

As HAC staff, your primary role is to assist people with disabilities who are eligible for HAC to find a home of their own. Your main responsibilities fall into three large areas, or rings. Promotion, Process, People.

PROMOTION

  • Work to successfully market, promote and implement HAC.
  • Establish effective working relationships with community partners around HAC.

PEOPLE

  • Provide information, referral and determine eligibility of people seeking HAC .
  • Provide direct services to individuals by helping them:
    • Seek and locate safe, affordable, accessible housing that is not owned, operated or leased by a provider of service.
    • Agencies and staff must not have a direct or indirect financial interest in the property or housing the participant selects.
    • Partner with their county or tribal nation to plan for a home of their own with the supports they need.
    • Know the rental properties where people without disabilities live in your service area.
    • Identify vacancies and properties matching the budget and desires of the people using HAC.
    • Consider a broad spectrum of issues related to housing.
    • Review and help complete rental applications and lease agreements.
    • Apply for publicly financed housing.
    • Develop household budgets.
    • Find affordable furniture and household goods.
    • Pack and move to a home of their own.
  • Meet with landlords/property staff.
  • Upon request, provide on-going assistance with housing related matters
  • Follow up with people on their questions or assistance if needed. If the person desires, be the initial liaison to support positive interactions with landlords/property owners.
  • Develop a short journal of each participant's relocation journey.

PROCESS

  • Comply with all policies and procedures as instructed.
  • Provide appropriate HAC activity, budget and other documentation and evaluation as required.
  • Establish and maintain records for each participant that includes: Name, date of birth, starting and ending address, county, phone number, and Medical Assistance (MA) home care or non-residential waiver service. Click Resources for forms and more information.
  • Maintain privacy at all times when providing outreach or having discussions with those other than the person receiving services.
  • Track the use of federally funded disability housing vouchers.
  • Track the use of housing assistance funds and a range of information including the DHS programs people use to get the support they need.
  • Follow HAC standards including having:
    • A current driver license to seek available housing, attend meetings and promote relationships.
    • Current automobile insurance.
    • Current background check
    • Passed no-cost DHS HAC eLearning course.
    • Completed no-cost DHS Vulnerable Adult Mandated Reporting online training.
