Prioritization and Relevance
As part of the golden thread, all related assessment data contributes to the development of a service plan which in turn sets the course of services and interventions. All parts of the process and the corresponding documentation incorporate the consumer as a full partner and consider the consumer’s choice, preference and desires as well as balancing risk.
Those domains in which a consumer’s status and function is impacted by how the consumer experiences their mental illness may be considered for potential behavioral health interventions and supports.
