Diagnostic Assessment. A Diagnostic Assessment
identifies the illness and explains how it is affecting the person.
Functional Assessment. A Functional Assessment
identifies the areas of daily living where essential skills need to be
developed or restored.
Level of Care Assessment. A Level of Care (LOC)
Assessment is used to determine what the appropriate level of service
would be for a given person.
SNAPP. A SNAPP reflects the person’s perspective
of his or her
