Regulations for Health Service Records
In accordance with Medicaid and state record keeping requirements, health service records must be developed and maintained as a condition of payment by Minnesota Health Care Programs (MHCP).
Each time a health service is delivered, it must be documented in the recipient's health record. MHCP funds paid for health care not documented in the health service record are subject to monetary recovery.
