Good assessments and service plans point specifically to the desired results of rehabilitation. They outline the functioning to be strengthened and the interventions that will improve functioning.

Strong progress notes are specific to the actual rehabilitation interventions and the recipient’s response, but sparse in describing conversations.  

Progress notes lend themselves to concurrent documentation and as evidence of progress toward goals and objectives.
