Summary

Progress notes serve as primary communication tools and as support of billing to MA.

They are much easier to write when good assessments and service plans point specifically to the desired results of rehabilitation by outlining the functioning to be strengthened and the interventions that will improve functioning.

Strong rehabilitation notes:

  • Specific as to the actual rehabilitation interventions
  • Recipient’s response
  • Sparse in describing conversations

Good progress notes lend themselves to concurrent documentation and as evidence of progress toward goals and objectives.
