Service Delivery

three file foldersThe assessment and the service plan provide the foundation for the main course, “service delivery”.

Documenting service delivery has several purposes:

  1. It communicates to everyone involved in coordination and delivering care exactly what happened in any given session, thus promoting communication and coordination of care.  The next session can begin where the last session concluded.
  2. The notes document the progress over time that the recipient has made toward achieving and sustaining functional improvements.   Progress notes serve as an evaluation of effective and ineffective interventions.
  3. The progress notes support billing and provide documented evidence of services delivered when audited.
