Module 4 continues the review of the MH-TCM model with its four core service components; and goes into depth on two of the components: referral and linkage, and monitoring and coordination.
The module provides introductory information and links to common statewide resources and services available to many clients of TCM services. Additionally, the module covers the documentation of adult MH-TCM services.
