  1. Discuss with your clinical supervisor the documentation standards and practices described in this curriculum section. Compare your agency’s documentation practices with these training practices.  Address these questions:
    • Do the client files of your agency contain the recommended elements that this agency suggests?
    • Is your agency able to obtain copies of current client diagnostic assessments and maintain DAs in client files?
    • Is your documentation reflecting the four core service components of Adult MH-TCM?
    • Do your functional assessments include narrative on each of the 11 functional domains for the client?
    • Does your agency encourage the use of concurrent documentation?
    • Is your clinical supervisor documenting client-specific and non-client specific clinical supervision that you are required to receive?  Is the clinical supervisor approving and signing FA’s and ICSP’s?
    • What resource/service gaps that exist in your community for case management clients?  What, if any, efforts are being made or might be made to address these gaps and develop new resources and services?

Document, date and save the summary of discussion of these questions in your training file.

  1. If you and your supervisor identify that you want to further develop your knowledge and skills in client crisis assessment and planning, take the Adult Mental Health Crisis Intervention Curriculum online training available on the DHS Adult Mental Health Division training site. This course takes about 6 hours to complete, but does not have to be completed all in one sitting. Although its primary target audience is Crisis Response Services personnel, it is of value to other mental health provider staff responsible to assist consumers who may be experiencing a mental health crisis.

Successful completion of this training curriculum will be recorded.

  1. For the two hypothetical client cases (Maria, Paul and Hassan) that you wrote ICSP’s for previously, identify and document real services and resources that you might refer these clients to in their communities. Utilize at least three of the Community Resources links identified above to assess information about resources/services.
