Diagnostic assessment and updates
A copy of the client’s current comprehensive diagnostic assessment and any updates to it must be in the client’s file. This document is part of the basis for eligibility for adult MH-TCM and must be used in the assessment and development of the client’s ICSP. If for some reason, the agency does not have current diagnostic assessment record, be sure to record where that information exists and how the case manager and clinical supervisor access/have accessed that information in the assessment and planning activities of the case management services.
A diagnostic assessment is the most common document where there is documentation of the requirement that a mental health professional verifies that the client meets the eligibility requirement of having a serious and persistent mental illness. Redetermination of continuing eligibility for case management must be documented in the client file.
