stack of papersFunctional assessment reviews provides the opportunity to add detail learned over the past period, as well as to assess and document the client’s current functioning.  The review focuses on the information that has changed or been expanded during the past period, rather than repeating history that has not changed.

It is especially important to update the functional assessment.  As a result of implementation of the ICSP and the client’s utilization of accessed services, resources, and natural supports. There is an expectation that a client’s functioning will change over the course of 90-180 days.

Incorporating both the case manager’s and the client’s perceptions of what has changed can be a powerful recovery reminder for all involved.
