Clinical supervision documentation
The client file should include the signature of the clinical supervisor approving the:
- Client’s functional assessment.
- Client’s ICSP.
- Written summary of client case consultation and reviews with case manager.
- Changes to client’s ICSP.
- Recommendations and directives concerning ongoing services.
The case manager’s personnel file should include documentation by the clinical supervisor:
- Signature on written summaries of required clinical supervision sessions with case manager.
- Documentation of required frequency and content of clinical supervision of case manager.
- Caution: Client specific identification information should NOT be noted in the staff personnel files.
- Signature of the clinical supervisor on the ICSP or any client file entry or supervisee personnel file entry, indicates that the clinical supervisor reviewed, authorized and approved.
