The client file should include the signature of the clinical supervisor approving the:

  • Client’s functional assessment.
  • Client’s ICSP.
  • Written summary of client case consultation and reviews with case manager.
  • Changes to client’s ICSP.
  • Recommendations and directives concerning ongoing services.

The case manager’s personnel file should include documentation by the clinical supervisor:

  • Signature on written summaries of required clinical supervision sessions with case manager.
    • Documentation of required frequency and content of clinical supervision of case manager.
    • Caution: Client specific identification information should NOT be noted in the staff personnel files.
  • Signature of the clinical supervisor on the ICSP or any client file entry or supervisee personnel file entry, indicates that the clinical supervisor reviewed, authorized and approved.
