Case Management in Minnesota
Case management is the provision of services that help individuals access needed medical, social, educational and other services. The type of case management provided is determined by the needs and characteristics of the persons served and types of services used.
The following list identifies types of case management services in Minnesota:
- Child Welfare Targeted Case Management (CW-TCM)
- Mental Health Targeted Case Management (MH-TCM)
- Relocation Service Coordination (RSC)
- Rule 185 Case Management for Persons with Mental Retardation or Related Conditions
- Vulnerable Adult/Developmental Disability Targeted Case Management Services (VADD-TCM)
- Waiver Case Management/Service Coordinator
