Roles of the Case Managers
Case managers perform critical roles in the lives of persons to whom they provide services. There are a number of roles that case managers perform:
Case managers assist in the role of advocates to assure persons receive appropriate services and support persons' self-determination and autonomy. As advocates, case managers inform policymakers about unmet needs of the persons they serve and work to modify or create new services.
Case managers connect persons to community resources . A familiarity of public and privately funded community services is a critical component of case management. Local food shelves, recreational activities, transportation options, medical specialists are examples of resources case managers use.
Case managers act as counselors when persons first seek help or are in crisis. Case managers listen to what persons and their families need, offer them support and provide information to help them make informed choices from available options.
