Case Management in Minnesota
Because of federal reimbursements, case managers must work closely with county supervisors and fiscal management teams to use the correct case management types when recording time and billing for case management activities.
Within parameters of state law, counties are responsible for administrative and service-related case management activities. Regardless of the type of case management, there are common case management functions or processes:
- Intake, eligibility determination and screening (administrative)
- Plan development, service authorization and plan implementation (both)
- Plan monitoring, evaluation and revision (service-related)
These case management processes are dependent on each other or inter-related. The success of case management depends on the quality and outcomes of these processes.
