Fundamentals of Case Management: Eligibility determination
Functional assessment : Process of identifying:
- Person's strengths, preferences, functional skills and needs for service and supports based on observations of performance in settings in which they normally occur.
- Extents to which natural supports and informal providers are able to meet persons' needs for support and services.
- Extents to which human services agencies and providers are able to provide or develop needed support or services.
Informal diagnostic evaluations: Rely on past performance such as school records, treatment data and social contacts that reflect performance.
Other types of assessments, such as social history assessments, can be completed by case managers. Depending upon the program/service area, different assessment forms may be required. These forms are typically completed with the input from persons who know the person.
