Case managers match information collected during the intake step with eligibility criteria of programs or service types that best meet persons' needs. Persons' may be eligible for more than one or none of the programs they express interest in.

Case managers are required to provide information on all available services and help persons make informed decisions among the programs/services. Once persons are found eligible for services, case managers must

  • Notify them of their eligibility.
  • Provide them with information on all services for which they qualify.
  • Assist them in making informed choices from among feasible alternatives.
