Fundamentals of Case Management: Screening
Screening is the last step in the intake process. Case managers use screening processes to determine levels of care for persons who request specific services such as home and community-based waivers or institutional placement by addressing needs, diagnoses and functional capacities.
The level of care assigned to a person by the screening team is reflective of the type of facility that can deliver that type of care. The four levels of care most commonly referenced are:
- Hospital
- Intermediate Care Facility for Persons with Mental Retardation or Related Conditions (ICF/MR)
- Neurobehavioral hospital (NB)
- Nursing facility (NF)
