Fundamentals of Case Management: Screening

Case manager roles and responsibilities will differ across screening processes depending on the program and/or service area in which they work. Roles common across case management activities are:

  • Convene screenings
  • Complete required paperwork
  • Enter screening information into the Minnesota Medicaid Management Information System (MMIS)

Screening teams

Screening teams are interdisciplinary groups of people that come together to review intake materials and assess the recipient needs. Persons seeking services are a vital part of this team, as well as family and/or their legal representatives.

There are specific roles, responsibilities and timelines that screening teams must adhere to. Case managers convene the screening teams as needed.
