Fundamentals of Case Management: Screening

Screening process

There are common aspects of the screening process that must be consistently handled across programs and services:

  • Persons who are being screened and their legal representatives are to be present at the screenings whenever possible.
  • Individuals who best know persons being screening should be present at the screenings.
  • Everyone present at the screenings understands the purpose of the screening and is encouraged to answer questions accurately and honestly.
  • Staff completing screenings are trained in specific screening documents and have knowledge how to code the document properly based on responses to the questions provided.
  • Information from screening documents is entered into the MMIS system accurately and in a timely manner.
