Fundamentals of Case Management: Screening

Case managers are responsible for completing screening documentation. Screening documents require some general information that case managers might have, such as demographic information and history. As case managers prepare for screening meetings, areas of the screening documents may be completed prior to the meetings. Other information cannot be completed until the teams meet, exchange and review assessment and evaluation materials.
Case managers complete screening documents the first time persons are assessed for or request home and community-based waivers or institutional placements , and when there are changes in the person's life that necessitate a full team review.
Upon completion of the screening documents, results are entered into the Minnesota Medicaid Management Information System (MMIS). As with the screening teams, there are specific roles, responsibilities and timelines that must adhered to regarding documentation.
