Plan development, service authorization and plan implementation
Once persons are eligible for Medical Assistance services through the intake processes, case managers are responsible for the creation of plans that guide in developing and coordinating of services, authorization of services and their implementation. These are separate but interrelated steps.
Plan development: Case managers use assessment information to help persons select services based on their needs, eligibility, cost effectiveness and preferences. Formal plans of service, signed by service recipients, legal representatives and case managers will identify the type, amount, duration and scope of services.
Service authorization: Case managers follow county procedures to enter services identified in the formal plans of service into the Minnesota Medicaid Management Information System (MMIS).
Plan implementation: Case managers help persons to identify providers, initiate and coordinate authorized services.
