Plan development

While case managers are responsible for the completion of service plans and all their required components, the process of developing plans is a collaborative one. Case managers rely on the input of persons eligible for services, their family, friends and legal representatives to provide important insight, strategies and resources to meet assessed needs.

The following list identifies key components of service plan development:

  • Assuring that persons are given opportunities to express needs and preferences
  • Providing ample information from which persons make choices regarding services
  • Assuring that health and safety needs are met
  • Incorporating results of assessments and screenings into a single, comprehensive, written plan that guides service providers and others in meeting health and safety needs
  • Providing for implementation and monitoring
