Plan development

While exact components and formats of plans may vary, the most critical components of plan development include the following.

  • Summaries of intake results (assessments, needs and preferences)
  • Expected outcomes and measurable goals directly related to assessed needs and preferences
  • What services will be provided and by whom
  • How and when services that are currently unavailable will be located

In some instances where serious health conditions are involved, reviews of plans by health care professionals may be necessary to be certain that services are sufficient and likely to be successful.

In some instances when county case managers are not responsible for developing plans, such as in Consumer Directed Community Supports (CDCS), case managers are still required to review and approve plans.
