Plan development: Service selection
Next is the completion of the service plans. This is the process where case managers take information learned about persons during the intake processes and planning meetings to identify and matches services to meet those assessed needs.
The types of services and supports identified in service plans are based on person's needs, regardless of the availability of these services. When it is not possible to meet all the service needs identified on service plans, document why the needs are not able to be met and outline a plan for meeting the needs in the near future.
When prioritizing service needs within service plans, critical considerations for county case managers include the following:
- Appropriateness of services (eligibility and cost effectiveness)
- Agreement to service plans by persons and their families/legal representatives
- Service plans meet health and safety needs
