Once the plan outlines the needed services and supports, identify the amount, duration and scope of services to be provided that are agreed upon by the team to achieve the identified goals and objectives. These services are specific to the short-term goals. As each of the short-term goals is met, the results feed into the achievement of the long-term goal(s).

Incorporate results of assessments and screenings into the plans. Obtain signatures of the service recipients, their family members and/or legal representatives as appropriate. Make copies of the plans for members of the team. After developing the service plans, case managers locate and authorize appropriate services and supports for the person.
