Let’s apply what you have learned to our two training cases. Let’s look at the Sinclair Jefferson training case first.

Sinclair requested MA-LTC on the first of this month. He reports that his wife, Mary, owns an annuity.

To Do: Based on the information provided in Mary’s annuity contract (PDF) determine whether or not an annuity transaction has occurred on Mary’s annuity.

Yes, an MA-LTC annuity transaction occurred on Mary’s annuity. The annuity was purchased one year ago on March 1.

The purchase of an annuity is a qualified action.
The purchase occurred during Sinclair’s lookback period.

To Do: Update Mary’s Annuity Data Worksheet (DHS-5532) to indicate that an annuity transaction occurred on 3/1/CY-1. This can be recorded on the “Annuity Transaction” line found in the “MA-LTC Information” section of the form. You could also indicate the type of transaction.
