Annuity Flowchart 1 – MA Process.
This flowchart reviews the MA annuity requirements. You must always review all annuities as a possible income source and/or available asset before determining if the annuities meet the MA-LTC requirements. Take a minute to review this flowchart.
This flowchart directs you to Annuity Flowchart 2 – MA-LTC Process when an applicant or enrollee is requesting MA-LTC.

Annuity Flowchart 3 – PRB.
This is a three page flowchart that walks you through the conditions that must be met to require an applicant or their spouse to designate DHS as a preferred remainder beneficiary on an annuity. It also walks you through the process of requesting completion of the Annuity Beneficiary Designation Form (DHS-5036) and the Issuer of Annuity Notice of Obligation (DHS-5037).
Then the flowchart directs you to the next page when the DHS-5037 has been returned with the next steps to follow and then takes you back to Annuity Flowchart 2 - MA-LTC Process or if the DHS-5037 is not returned it takes you directly back to the Annuity Flowchart 2 – MA-LTC Process.
