Question 1.

Yes, Padma receives $678.00 each month. If you are unsure how this was determined please refer to the Annuities 101 course.

Question 2.

No, the annuity has been annuitized and there is no cash value. If you are unsure how this was determined please refer to the Annuities 101 course.

Question 3.

Yes, Padma completed and returned the Required Annuity Information form (DHS-5143). She also provided a copy of her annuity contract and statement which provides information on the annuitization of this annuity, which is an MA-LTC annuity transaction and was discussed in a previous module.

Question 4.

Yes, Padma is required to designate DHS as the PRB on her annuity in order to be eligible for MA-LTC because she met the three conditions:

Condition 1 - Padma is requesting MA-LTC and is the owner of the annuity.
Condition 2 - Currently Padma’s estate is the beneficiary of her annuity. The annuity contract does not indicate that only Aman (Padma’s husband) can be designated as a beneficiary. This condition will be confirmed later from the DHS-5036.
Condition 3 - In Module 3, Annuity Transactions, you determined an MA-LTC annuity transaction occurred with the annuitization of Padma’s annuity on the 30th of the current month (CM) minus 8 months.

Question 5.

Send Padma the MHCP Request for Information (DHS-3271) requesting she complete and return the Annuity Beneficiary Designation form (DHS-5036) within 10 days. Set a MAXIS DAIL/WRIT with the expected date of return.

Question 6.

You send Padma’s annuity issuer (Lincoln National Life Insurance Company) the original DHS-5036 signed by Padma, along with a completed Issuer of Annuity Notice of Obligation (DHS-5037) after making copies of all of the forms for your case file. Set a DAIL/WRIT for 30 days.

Question 7.

Send a copy of the completed DHS-5037 to the DHS Special Recovery Unit. Complete a transfer evaluation of the annuity.
