Remember, the service plan points to the kinds of interventions that are being used. The progress note specifically describes what the staff member did as an intervention.
The intervention described must be a covered rehabilitation intervention. The verbs in the sentences are often keys to describing the rehabilitation nature of the intervention. The verbs should tell the story of active work or actions taken that are aimed at how the recipient can restore functioning.
