Updating and Review

The chart outlines the required time frames for updating assessments and treatment plans.  These are minimum timeframes.  Assessments and treatment plans should be updated whenever there is a significant change in the recipient’s situation or illness.

The updating and review of assessments and treatment plans are much more than an administrative requirement. They serve as an important time to reflect on what is working, what the recipient wants in life as well as from rehabilitation, and what progress has occurred.

6 months – Note MHCP provider manual says 90 days with ARMHS allowing 180.

Qualified ACT team members prepare outcome-oriented rehabilitation plans.  Update them on a timely basis.  At least once every 6 months as a full team and monthly with each consumer.
