The treatment plan review should reflect the team’s perception and perspective of what has occurred over the past time period.
It is not a cut and paste of the six months of progress notes. It is the synthesizing, distilling and interpretation of what all those notes mean for the recipient and the team.”
The treatment plan review:
- Should address those things that have been the successes and barriers
- What is left to accomplish (keeping goals as is? Changing objective baselines and targets?)
- New priorities from the client
- New challenges observed or expressed by client
- Any additional or new preferences and what is next for the team
