Functional Assessment Review
The functional assessment review provides the opportunity to add detail learned over the past period, as well as to assess and document the recipient’s current functioning.
The review focuses on the information that has changed or been expanded during the past period, rather than repeating history that has not changed.

Functional Assessment
At minimum, FA is reviewed once every 6 months.
Assessment must be updated at least every 3 months or prior to discharge, whichever occurs first.
