For example, an ACT psychiatrist may have been seeing the recipient in the recipient’s home for much of the ACT service delivery.

As discharge nears, the psychiatrist (or counselor or nurse), may transition to seeing the recipient in an office setting, so that the recipient begins to gain the skills to manage outpatient appointments post-ACT. 

While discharge plans and goals reflect standardized service discharge criteria, they should be individualized and personalized for each recipient.
